Surgical Treatment of Intrahepatic Inferior Vena Cave Obstruction




Abstract
The Budd-Chiari syndrome is a rare type of portal hypertension caused by complete or incomplete
obstruction of the hepatic veins or the corresponding portion of the inferior vena cava or both.
In two adult women, the obstruction were located just beneath the diaphragm, above the right
hepatic vein opening, which were confirmed by vena cavography preoperatively.
Under the normothermia, using heart-lung machine, transatrial dilataion of the stricture in the
second case and removal of the thrombus in the first case were done.
The one is caused by thrombus of unknown primary origin and in the other, the cause of the
stricturies unknown.
Postoperatively, ther were marked fall of venous pressure and symptoms and signs improved
remarkably.
